T\u0131pk\u0131 \u00fclkenin her bir k\u00f6\u015fesini enkaza \u00e7eviren, do\u011fal tahribata yol a\u00e7an kentsel d\u00f6n\u00fc\u015f\u00fcm, termik santraller, HES\u2019ler, AVM\u2019ler gibi \u015eehir Hastaneleri de do\u011faya ve sa\u011fl\u0131\u011fa zararl\u0131d\u0131r. \u015eehir hastanleriyle geli\u015fen sa\u011fl\u0131kl\u0131 ya\u015fam hakk\u0131 de\u011fil ba\u015fta in\u015faat, finans, t\u0131p end\u00fcstrisi ve bili\u015fim olmak \u00fczere sermaye \u00e7evreleridir.\u00a0 Sa\u011fl\u0131k sorunlar\u0131n\u0131n \u00e7\u00f6z\u00fcm\u00fcnde \u00f6nceliklerin saptanmas\u0131, t\u0131bbi bak\u0131m hizmetlerin, devasa hastanelerin katk\u0131s\u0131n\u0131n tart\u0131\u015f\u0131lmas\u0131 gerekir ve karar s\u00fcre\u00e7lerinde halk\u0131n, sa\u011fl\u0131k emek\u00e7ilerinin, akademinin, sa\u011fl\u0131k emek ve meslek \u00f6rg\u00fctlerinin yer almas\u0131 gerekirken meclis, hatta bizzat AKP\u2019li milletvekilleri dahi karar s\u00fcre\u00e7lerinin d\u0131\u015f\u0131nda b\u0131rak\u0131lm\u0131\u015ft\u0131r. Keza uluslararas\u0131 olumsuz deneyimler de g\u00f6rmezden gelinmi\u015ftir. K\u0131ble sermayeye \u00e7evrilmi\u015ftir.<\/p>\n<p>\u015eehir hastaneleri i\u00e7in s\u00f6ylenen gerek\u00e7eleri kabaca \u015f\u00f6yle \u00f6zetleyebiliriz: N\u00fcfus art\u0131yor, n\u00fcfus ya\u015flan\u0131yor; bula\u015f\u0131c\u0131 olamayan hastal\u0131klar-kronik hastal\u0131klar (kalp-damar hastal\u0131klar\u0131, solunum sistemi hastal\u0131klar\u0131, kanserler) art\u0131yor; bununla birlikte tan\u0131 ve tedavi olanaklar\u0131 da art\u0131yor; sa\u011fl\u0131k okuryazarl\u0131\u011f\u0131 geli\u015fiyor, sa\u011fl\u0131k bilinci art\u0131yor ve vatanda\u015f\u0131n beklentileri farkl\u0131la\u015f\u0131yor; sa\u011fl\u0131k hizmetleri i\u00e7in yeni yat\u0131r\u0131mlara ihtiya\u00e7 var; y\u00fczy\u0131ll\u0131k k\u00f6hne hastanelerle bu i\u015f y\u00fcr\u00fcmez, olanaklar yetersiz\u2026 <span style=\"text-decoration: line-through;\">K<\/span>amunun finansman (kaynak, para) sorunu var; \u00fcstelik paray\u0131 kullanmay\u0131 da bilmiyor; yat\u0131r\u0131mlar daha uzun s\u00fcrede ve daha pahal\u0131ya bitiyor. Bu sorunlara panzehir Kamu-\u00d6zel Ortakl\u0131\u011f\u0131\u2019d\u0131r. Peki ger\u00e7ekten \u00f6yle mi? \u015eehir hastaneleri derdimize \u00e7are mi?<\/p>\n<p><strong>Hastane gereksinimine sermaye ve iktidar el ele karar veriyor<\/strong><\/p>\n<p>\u015eehir hastaneleri ile di\u011fer hastane projelerinin aras\u0131ndaki temel fark Kamu-\u00d6zel Ortakl\u0131\u011f\u0131 modeli ile yap\u0131l\u0131yor olmalar\u0131. \u00c7al\u0131\u015ftay\u0131m\u0131z bunun Devlet-Sermaye i\u015fbirli\u011fi olarak adland\u0131r\u0131lmas\u0131n\u0131n daha anla\u015f\u0131l\u0131r olaca\u011f\u0131n\u0131 kan\u0131s\u0131na vard\u0131. Yukarda bahsedildi\u011fi gibi g\u00f6z dikilen kamunun-toplumun kaynaklar\u0131 olmas\u0131na ra\u011fmen karar alma s\u00fcre\u00e7lerine toplumunkat\u0131l\u0131m\u0131na izin verilmemi\u015ftir. AKP\u2019nin ya\u015fam\u0131n her alan\u0131nda ortadan kald\u0131rd\u0131\u011f\u0131 demokrasi, burada da rafa kald\u0131r\u0131lm\u0131\u015ft\u0131r. Sermaye ile birilerinin hayalleri u\u011fruna kamu-toplum kaynaklar\u0131 pe\u015fke\u015f \u00e7ekilmi\u015f, \u00e7ekilmeye devam edilmektedir. K\u00d6O sayesinde devlet ile sermaye aras\u0131ndaki s\u0131cak ili\u015fkiler t\u00fcm \u00e7\u0131plakl\u0131\u011f\u0131 ile bir kez daha g\u00fcn y\u00fcz\u00fcne \u00e7\u0131km\u0131\u015ft\u0131r. Devlet sermaye i\u015fbirli\u011fi ile hedeflenenin devletin planlama ve sa\u011fl\u0131k emek g\u00fcc\u00fc ve \u00f6zel sekt\u00f6r\u00fcn finansman ve emek g\u00fcc\u00fcn\u00fc organize etme becerisinin birle\u015ftirilmesi diye dile getiriliyor. Devasa \u00f6l\u00e7ekli yat\u0131r\u0131mlar, sermayenin i\u015ftah\u0131n\u0131 kabartan, hayallerini s\u00fcsleyen, pazar garantili yat\u0131r\u0131m olanaklar\u0131 sermayenin hizmetine sunuluyor. Sermayenin hayali emek\u00e7ilerin eme\u011fi ve vergileriyle olu\u015fturulan b\u00fct\u00e7e-kamu kaynaklar\u0131n\u0131n istismar\u0131d\u0131r. \u00d6zel sekt\u00f6r\u00fcn emek s\u00fcrecindeki avantajlar\u0131 ya da kapasitesi denildi\u011finden ne \u00e7\u0131kt\u0131\u011f\u0131n\u0131 yine en iyi emek\u00e7iler bilmektedir: S\u00f6m\u00fcr\u00fcn\u00fcn derinle\u015fmesi ve emek ya\u011fmas\u0131. Sermayenin hayalleri, sa\u011fl\u0131k emek\u00e7ilerini kabusu \u015feklinde kar\u015f\u0131l\u0131k buluyor.<\/p>\n<p><strong>Sermayenin ars\u0131zl\u0131\u011f\u0131, Devlet-Sermaye \u0130\u015fbirli\u011finin hukuksuzlu\u011fu, denetimsizli\u011fi ve keyfili\u011fi: Devasa hastaneler, devasa \u015firketler, devasa s\u00f6zle\u015fmeler, devasa yolsuzluk&#8230;<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p>Sermayenin hayallerini ve T\u00fcrkiye \u00f6zg\u00fcnl\u00fc\u011f\u00fcnde Devlet-Sermaye \u0130\u015fbirli\u011fi ile yap\u0131lan \u015eehir Hastaneleri mega projelerini biraz daha e\u015felemekte yarar var. T\u00fcrkiye modelinde pervas\u0131zl\u0131k, ars\u0131zl\u0131k ve denetimsizlik \u00e7ok fazlad\u0131r. Bunlar\u0131 \u015fu \u015fekilde s\u0131ralayabiliriz: \u015eehir hastanelerinin say\u0131s\u0131n\u0131n olduk\u00e7a fazla olmas\u0131; \u00e7o\u011fu hastaneden yap\u0131lacak b\u00f6lge i\u00e7in \u00c7ED raporu (\u00e7evresel etki de\u011ferlendirmesi) istenmemesi, \u015fehir planlamas\u0131na dikkat edilmemesi; \u015firketlerle yap\u0131lan ihale ve s\u00f6zle\u015fmelerin kamu ihale kanunu kapsam\u0131nda olmamas\u0131, kamu denetimine y\u00fck\u00fcml\u00fc kurumlar\u0131n (TBMM, Say\u0131\u015ftay vb) denetim alanlar\u0131n\u0131n s\u0131n\u0131rl\u0131 olmas\u0131; ihale s\u00f6zle\u015fmelerinin \u015feffaf olmamas\u0131, ticari s\u0131r olarak kabul edilmesi ve kamuoyuna a\u00e7\u0131klanmamas\u0131; arazinin kamu taraf\u0131ndan tahsis edilmesi; projenin tasar\u0131m maliyetinin dahi kamu (Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131) taraf\u0131ndan \u00fcstlenilmesi; risk payla\u015f\u0131m\u0131 konusunda devletin rol\u00fcn\u00fcn b\u00fcy\u00fck olmas\u0131 (\u015firketin ald\u0131\u011f\u0131 finansman kredisini \u00f6demede g\u00fc\u00e7l\u00fck \u00e7ekti\u011finde kamu taraf\u0131ndan \u00f6denmesi), 25 y\u0131ll\u0131k i\u015fletme s\u00fcresi boyunca, Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131\u2019n\u0131n \u015firkete y\u0131ll\u0131k kira \u00f6deyecek olmas\u0131, bunun yan\u0131 s\u0131ra, Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 taraf\u0131ndan garanti edilen hizmet \u00f6demelerinin %70 doluluk oran\u0131na dayal\u0131 olmas\u0131, \u00f6demelerin d\u00f6viz kuru, \u00dcFE ve T\u00dcFELER \u00fczerinden olmas\u0131 ve bunlara asgari \u00fccret art\u0131\u015flar\u0131n\u0131n dahi eklenmesi, kamunun \u00f6zel ad\u0131na bor\u00e7lanmas\u0131, vergiden muaf olmas\u0131; b\u00fct\u00e7ede bu harcamalar\u0131n g\u00f6r\u00fclmemesi; kapanan hastanenin cihazlar\u0131n ve demirba\u015f gibi malzemelerin \u015fehir hastanelerine ta\u015f\u0131nmamas\u0131-donan\u0131m\u0131n t\u00fcm\u00fcyle yeni olmas\u0131 ve hastanelerin \u00e7ok b\u00fcy\u00fck \u00f6l\u00e7ekli (yatak say\u0131s\u0131, kapal\u0131 alan) yap\u0131lmas\u0131, bunlar\u0131n \u00e7o\u011funlu\u011funun \u015fehrin d\u0131\u015f\u0131na yap\u0131l\u0131yor olmas\u0131, dolay\u0131s\u0131yla bu alanlar\u0131n kentsel ranta d\u00f6n\u00fc\u015ft\u00fcr\u00fclmesini getirmesi, yap\u0131lal\u0131 veya onar\u0131lal\u0131 hen\u00fcz 5 y\u0131l dahi olmam\u0131\u015f devlet hastanelerinin bile kapat\u0131larak kamunun zarara u\u011frat\u0131lmas\u0131, ayr\u0131ca \u015eehir Hastanesi\u2019ne ta\u015f\u0131nan hastanelerin bir k\u0131sm\u0131 E\u011fitim Ara\u015ft\u0131rma Hastaneleri olmas\u0131na ra\u011fmen ne e\u011fitim ne de ara\u015ft\u0131rma i\u00e7in fiziksel altyap\u0131 dahi d\u00fc\u015f\u00fcn\u00fclmemi\u015f olmas\u0131. T\u00fcrkiye \u00f6zg\u00fcnl\u00fc\u011f\u00fc a\u00e7\u0131s\u0131ndan \u015fehir hastanelerinin h\u00fck\u00fcmet yetkilileri taraf\u0131ndan sa\u011fl\u0131k turizmi ile birlikte an\u0131lmas\u0131n\u0131 da dahil etmeliyiz. Yap\u0131lan \u015fehir hastanelerin hizmete ge\u00e7mesi sonras\u0131 ya\u015fananlarla bu listeye yenileri de eklenecekgibi g\u00f6r\u00fcn\u00fcyor. T\u00fcm bunlar\u0131 T\u00fcrkiye ekonomisi i\u00e7in kilit rol oynayan emlak-in\u015faat sekt\u00f6r\u00fcn\u00fcn g\u00fcncel ihtiya\u00e7lar\u0131n\u0131n kar\u015f\u0131lanmas\u0131 olarak da okuyabiliriz. T\u00fcrkiye kapitalizmi i\u00e7in in\u015faata dayal\u0131 sermaye birikimi y\u00f6nl\u00fc tan\u0131mlamalar\u0131n yap\u0131ld\u0131\u011f\u0131n\u0131 da akl\u0131m\u0131z\u0131n bir k\u00f6\u015fesinde tutmal\u0131y\u0131z. Bu minvalde \u015fehir hastaneleri t\u00fcm i\u015f\u00e7i s\u0131n\u0131f\u0131n\u0131n ge\u00e7mi\u015fini-biriktirdiklerini, bug\u00fcn\u00fcn\u00fc ve gelece\u011fini gaspetmeye g\u00f6z diken mega projeler \u00f6zelli\u011fi ta\u015f\u0131maktad\u0131r..<\/p>\n<p><strong>Sermayeninin yeni de\u011ferlenme alan\u0131 olarak Sa\u011fl\u0131kta D\u00f6n\u00fc\u015f\u00fcm \u015eehir Hastaneleri ile devam ediyor<\/strong><\/p>\n<p>Sa\u011fl\u0131kta D\u00f6n\u00fc\u015f\u00fcm Program\u0131\u2019n\u0131n ikinci faz\u0131 olan \u015fehir hastaneleri hem sa\u011fl\u0131k emek\u00e7ileri hem de halk i\u00e7in, hatta ta\u015f\u0131nacak hastanelerin yan\u0131ba\u015flar\u0131ndaki bir \u00e7ok esnaf i\u00e7in ciddi sorunlar ta\u015f\u0131maktad\u0131r. Son elli y\u0131la yay\u0131lan reform (y\u0131k\u0131m) giri\u015fimleri (hastaneleri i\u015fletmele\u015ftirme, d\u00f6ner sermaye, hizmet sat\u0131n alma, alt i\u015fverene devredilen hizmetler, toplam kalite y\u00f6netimleri, genel sa\u011fl\u0131k sigortas\u0131, \u00f6zel sa\u011fl\u0131k sigortas\u0131, fiyatland\u0131rma-SUT uygulamalar\u0131, s\u00f6zle\u015fmeli \u00e7al\u0131\u015ft\u0131rma, kamu hastaneler birli\u011fi, \u00f6zel sa\u011fl\u0131k sekt\u00f6r\u00fcn\u00fcn d\u00fczenlenmesi, sa\u011fl\u0131k turizmi vb.) \u015fehir hastaneleri ile ta\u00e7land\u0131r\u0131l\u0131yor.<\/p>\n<p>T\u00fcrkiye\u2019de \u015fehir hastaneleri projeleri hastanenin yap\u0131m\u0131n\u0131, donan\u0131m\u0131 ve t\u0131bbi donan\u0131m\u0131 i\u00e7eren alt yap\u0131 projesi olman\u0131n yan\u0131nda bunun ile de s\u0131n\u0131rl\u0131 kalm\u0131yor, merkezi kapitalist \u00fclkelerdeki gibi tesisin bak\u0131m-onar\u0131m\u0131n\u0131n yap\u0131lmas\u0131, destek ve t\u0131bbi destek hizmetlerinin y\u00fcr\u00fct\u00fclmesi, ticari alanlar\u0131n i\u015fletilmesini de i\u00e7eriyor. Liste ana ba\u015fl\u0131klarla b\u00f6yle, alt ba\u015fl\u0131klar da s\u0131ralan\u0131nca \u00e7ok daha uzun bir liste kar\u015f\u0131m\u0131za \u00e7\u0131kar. \u00d6zellikle tesis bak\u0131m onar\u0131m\u0131, destek ve t\u0131bbi destek hizmetleri alt ba\u015fl\u0131klar\u0131n\u0131n \u00e7ok geni\u015f oldu\u011funu biliyoruz. Hastenelerin s\u00f6zle\u015fmelerine ula\u015f\u0131lamad\u0131\u011f\u0131 i\u00e7in bilinmeyen hizmetler de devrede olabilir, zaman i\u00e7inde g\u00f6rece\u011fiz. Ticari alanlarda durum daha belirsiz, \u015firket onay ald\u0131\u011f\u0131 her t\u00fcrl\u00fc ticari faaliyeti yapabilir. Bu i\u015fletme hakk\u0131 \u015fimdilik 25 y\u0131l, ama uzat\u0131labiliyor. T\u00fcrkiye\u2019de i\u015fletme hakk\u0131 25-49 y\u0131l aras\u0131nda oldu\u011fu g\u00f6r\u00fc\u015f\u00fc s\u0131k dile getiriliyor. Y\u00fcklenici \u015firket bu hizmetleri ve in\u015faati ikinci, \u00fc\u00e7\u00fcnc\u00fc alt i\u015fverenlere devredebiliyor.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Sa\u011fl\u0131kta fabrika rejimi: Etkinlik, rekabet ve verimlilik vatanda\u015f\u0131n istismar\u0131 ve sa\u011fl\u0131ks\u0131zl\u0131\u011f\u0131, sa\u011fl\u0131k emek\u00e7ilerinin s\u00f6m\u00fcr\u00fcs\u00fc ve emeklerinin ya\u011fmalanmas\u0131d\u0131r<\/strong><\/p>\n<p>\u015eehir hastanelerine sermaye neden yat\u0131r\u0131m yap\u0131yor? Bu sa\u011fl\u0131k emek\u00e7ileri ve halk i\u00e7in yan\u0131tlanmas\u0131 gereken kritik sorudur. En basit haliyle yan\u0131t\u0131 herkes biliyor: KAR. Peki bu kar\u0131n kayna\u011f\u0131 nedir, kar nereden elde edilecektir? O da belli: Sa\u011fl\u0131k emek\u00e7ileri ve Halk.<\/p>\n<p>\u00d6l\u00e7e\u011fin b\u00fcy\u00fct\u00fcld\u00fc\u011f\u00fc devasa \u015fehir hastanelerinde \u00f6zel sekt\u00f6r\u00fcn organizasyonel (\u00fcretim-emek s\u00fcreci) ba\u015far\u0131s\u0131 g\u00fcndeme gelecektir. Her derde deva olan sa\u011fl\u0131kta da kendine yer bulan yal\u0131n \u00fcretim \u015fehir hastanelerinde t\u00fcm \u00fcretim s\u00fcrecine hakim olacakt\u0131r. Yal\u0131n \u00fcretim \u2018<em>\u00e7o\u011fu azla yapma\u2019<\/em>ve \u2018<em>art\u0131de\u011fere d\u00f6nmeyen art\u0131k zamanlar\u0131n azalt\u0131lmas\u0131\u2019<\/em>\u00a0 gibi iki temel ama\u00e7 ile tan\u0131mlan\u0131yor. Bu ama\u00e7 daha \u00e7ok verimlilik ve etkinlik s\u00f6zc\u00fckleri ile savunula geliyor. Girdinin azalt\u0131lmas\u0131, \u00e7\u0131kt\u0131n\u0131n (kar\u0131n) art\u0131r\u0131lmas\u0131 ile m\u00fcmk\u00fcn oluyor. Sa\u011fl\u0131k emek\u00e7ileri, t\u0131bbi teknoloji, t\u0131bbi sarf malzemelerinin kullan\u0131m\u0131n\u0131n de\u011ferlendirilmesinde verimlilik kar\u015f\u0131m\u0131za \u00e7\u0131k\u0131yor. Bunlar\u0131n sa\u011flanmas\u0131 i\u00e7in sa\u011fl\u0131k emek\u00e7ilerinin \u015firketin istediklerine g\u00f6re \u00e7al\u0131\u015fmalar\u0131 istenmektedir. \u015eehir hastanelerinde daha fazla hasta bak\u0131lmas\u0131, daha fazla tetkik yap\u0131lmas\u0131, daha fazla t\u0131bbi giri\u015fim yap\u0131lmas\u0131 isteniyor. \u00d6zetle hastaneler fabrikalara d\u00f6n\u00fcyor, d\u00f6necek. Bir g\u00fcnde tamamlanamayan, \u00fcst\u00fcste gidilen g\u00fcnler de dikkate al\u0131nd\u0131\u011f\u0131nda bu ula\u015f\u0131labilirlik sorunun maddi boyutu da ciddi k\u00fclfet olu\u015ftuca\u011f\u0131 g\u00f6r\u00fclecektir. \u00d6zellikle bu sorun ya\u015fl\u0131, engelli, \u00e7ocuk ve kad\u0131n hastalar, uzak semtlerden gelen yoksul hastalar\u0131 ciddi olarak etkilemektedir. Ula\u015f\u0131labilirlik sorunu acil hastalar i\u00e7in de ciddi zaman kayb\u0131 demektir ve geri d\u00f6nd\u00fcr\u00fclemez sa\u011fl\u0131k sorunlar\u0131na da yol a\u00e7ma riski vard\u0131r. Hastane i\u00e7inde ula\u015f\u0131m i\u00e7in reklam edilen golf arabalar\u0131n\u0131 yakalamak olduk\u00e7a g\u00fc\u00e7 olmaktad\u0131r.
